Medical/Hospital Itemized Receipt
Medical itemized receipts (also called itemized bills) are different from retail receipts. They include:
- Procedure codes (CPT codes)
- Diagnosis codes (ICD-10)
- Individual charges for each service (exam, lab work, imaging)
- Insurance adjustments and what insurance paid
- Your out-of-pocket responsibility
Hospitals don't always send itemized bills automatically. You often receive a summary that says "Amount Due: $847." To get the itemized version, call the hospital's billing department and request an itemized statement. They're legally required to provide one. When You Need an Itemized Receipt
Itemized Receipt for Taxes
The IRS accepts itemized receipts as proof of deductible business expenses. To qualify, the receipt needs to show:
- The date of purchase
- The vendor/seller name
- A description of each item or service
- The amount paid
A credit card statement alone doesn't satisfy IRS requirements because it doesn't describe what you bought. "Office Depot - $234.56" could be anything. An itemized receipt from Office Depot showing "HP LaserJet Toner - $189.99, Copy Paper 10-ream - $44.57" proves the purchase was a legitimate business expense.

Itemized Receipt for Insurance Claims
Insurance companies require itemized receipts to process claims. Whether it's health insurance, auto insurance, or homeowner's insurance, the adjuster needs to see specifically what was purchased or repaired, not just a total.
For health insurance: request an itemized bill from your provider showing CPT codes and individual charges. The Explanation of Benefits (EOB) from your insurer is not the same thing. The EOB shows what they processed; the itemized bill shows what was charged.
Itemized Receipt for FSA/HSA Reimbursement
FSA and HSA administrators reject receipts that don't list individual items. A receipt that says "CVS Pharmacy - $28.50" won't be approved. They need to see "Advil 200mg 100ct - $12.99, Band-Aid Flexible Fabric - $5.49, Saline Nasal Spray - $10.02" to verify each item is FSA/HSA eligible.

Most corporate expense policies require itemized receipts for purchases over $25-$75 (the threshold varies by company). A credit card statement isn't enough. Your finance team wants to verify that the $85 lunch was a team meal, not a personal shopping trip.
