The GOAT template separates Bill To from Ship To above an itemized table. Use the same-address option when both destinations match, or enter them separately. Product rows calculate quantity times unit price. Tax and shipping are each entered as a percentage of the item subtotal.

Is Shipping / handling a fixed currency amount?

No. It is a percentage of the item subtotal. For example, 5% of a $200 subtotal adds $10. Check that the resulting shipping amount matches your record.

Where do I put sneaker size or condition?

Include those details in the product description. The table has no separate size or condition selector.

Does this invoice verify a GOAT purchase or product?

No. It formats your entries and does not connect to GOAT orders or authentication. Obtain the original record through the marketplace.
