Keep the purchase and item details together
The Kay Jewelers template has a purchase-information block followed by customer fields, an item list and payment details. Each section opens independently. Use the item descriptions for the pieces being recorded and keep any separate service charge on its own line.
Working through the sections

| Customer | Change the two-column labels and values to match the information you need. |
