The source has no separate currency field. Include the currency in your unit-value and total-value entries. The fictional example uses USD; changing those labels does not convert amounts.
Yes. The optional business-logo upload uses blank upper-right space on both pages. The carrier-published form itself has no UPS logo in that area.
