This USPS return receipt template uses the complete two-sided July 2015 PS Form 3811 sample. It records delivery details and the address for returning the card. It is different from PS Form 3800, which records Certified Mail fees, and it contains no payment or postage-total section.

The source has a blank article-number line and a separate preprinted sample return-receipt number on both faces. The barcode images are independent controls. SAMPLE and the strike-through marks remain in the starting layout and example. The delivery signature area stays blank until you supply your own image.

PS Form 3811 is the Domestic Return Receipt card for delivery details and return-to-sender addressing. PS Form 3800 records mailing fees and recipient information. They are separate physical forms with different fields.

SAMPLE and the crossed-out barcodes are part of the selected historical reference. They are preserved on both faces; address fields occupy the clear space above that printing.
No. The article number transferred from the service label is separate from the return-card number printed on both faces. Barcode images are also independent and do not regenerate from either text field.
