Dental Receipt

Dental procedures are coded using the ADA Current Dental Terminology (CDT) system. Codes begin with D followed by four digits, such as D1110 for an adult cleaning or D2750 for a porcelain crown. These codes are required for insurance claims and FSA or HSA reimbursement.

Yes. Most FSA and HSA plans accept dental receipts that show the provider name, date of service, type of service, and amount paid. A general payment summary or credit card statement alone is usually not sufficient — you need an itemized receipt.

Contact your dental office directly and request an itemized statement or receipt for the date of service. Most practices can generate one from their billing system. If the office has closed, your dental insurance carrier may have an explanation of benefits (EOB) on file that serves a similar purpose.

A dental receipt confirms that you paid the provider and shows the amount you paid out of pocket. An EOB is sent by your insurance company and shows what the insurer paid, what was adjusted, and what portion was your responsibility. For reimbursement purposes, some plans accept either; others require both.
