Page two breaks out taxable value, CGST, SGST and cess, then prints the salesperson, transaction and invoice identifiers. No prescription measurements are printed in this invoice.

No. It follows a tax invoice for the optical purchase. The separate customer order sheet reviewed during research contains prescription measurements, but those unprinted sections are not added here.
Yes. Both card amounts, masked endings and authorization references are editable. The printed Balance Due label is retained before the payment rows, as in the source.
All amounts are manual. The example uses INR 6,000 in listed item amounts, INR 1,800 discounts and INR 4,200 total, including INR 450 GST. The two card payments add to INR 4,200.

The original print breaks that heading at the page boundary. The output places the complete heading above the continuation table while retaining every tax row and the original two-page layout.
